Zoom 15 at latitude 0 and 96 DPI gives about 4.78 metres per pixel and a scale of about 1 : 18,056.

This is a web-mercator approximation. It does not account for a specific map provider, tile size other than 256 px, or printed output size.

Why does latitude change the scale?

In web mercator, ground resolution depends on cos(latitude), so the same zoom covers less ground near the poles.

Is this an exact printed scale?

No. It is an estimate at the DPI you enter.
